Abstract

Asthma is a chronic respiratory disease characterized by episodes or attacks of impaired breathing. Symptoms are caused by inflammation and narrowing of small airways and include shortness of breath, coughing, wheezing, and chest tightness. Adiponectin is a protein specifically secreted from adipose tissue . It circulates to influence other organs as the liver, skeletal muscles, and blood vessels. An auto/paracrine effect on adipose tissue also exists. It has antidiabetic (by promoting insulin sensitivity), anti-inflammatory and anti-atherogenic effects. Its secretion is influenced by different hormones and cytokines. Hypoadiponectinemia is observed in obesity, type 2 diabetes, hypertension, coronary artery disease and bronchial asthma. In the present study was aiming to measure serum adiponectin concentration as a marker of severity in asthmatic children. The study included eighty children, aged from six to twelve years fifty out of them were suffering from bronchial asthma (obese cases ”n=12” & non obese cases ”n=38”). The remaining thirty were healthy age and sex matched children taken as a control group. Each child in this study was subjected to: 1- Full history taking. 2- Full clinical examination. 3-Anthropometric measurements in form of: (Weight, Height, BMI, waist circumference, hip circumference and waist-hip ratio). 4- Pulmonary function test in the form of PEFR. 5- Estimation of serum adiponectin level by (ELISA). Statistical analysis was done, our results revealed that: The sex distribution in our study revealed male predominance (52%).our results showed that increase in prevalence of asthma among rural than urban areas . The results showed that the severity of asthma non significantly increased with increasing BMI.
In the study, we found that our patients had a highly significant
decrease in PEFR level when compared to the controls. In asthmatic
group, PEFR highly significant decreased with increasing severity of
asthma. also PEFR highly significant decrease in obese than non obese cases . The study showed that in both gender serum adiponectin levels of asthmatic children ( obese & non obese ) were significantly decreased than those of healthy control. In the study, there was a non significant decrease in adiponectin level in obese than non obese cases. Also serum adiponectin was higher in girls than boys in patients and controls. The study, showed that serum adiponectin level non significant decrease with increasing asthma severity. the results showed that there was a significant negative correlation between adiponectin level and BMI and a significant positive correlation between adiponectin levels and PEFR.
