Abstract

This thesis was conducted in department of neurosurgery in Minia University Hospitals on thirty patients. Twenty one patients were males and 9 patients were females. The age of the patients ranged from 5 to 70 years and the mean age ± SD equalled 35.47 ± 17.91 years and the patients were classified into three groups; less than 16 years old, between 16 and 40 years old and above the age of forty years old. Sixty percent came due to MVA, 30% came due to FFH and 10% came due to assault from other. Males constituted 66.67% of patients came due to MVA and 33.33 % were females while 77.78% of patients who came due to FFH were males and 22.22% were females. 66.67% of patients came due to AFO were males and 33.33% were females.
56.67% had complained from headache, 43.33% had a complaint of vomiting and 36.67% had both complaints. 36.67 percent had a GCS less than 9 and 63.33% had a GCS more than 8. Other complaints were right hemiparesis in 16.67%, left hemiparesis in 20%, quadriparesis in 6.67%, aphasia in 3.33%, neck pain in 13.33%, neck stiffness in 33.33%, abnormal pupils in 30% and convulsions in 16.67% of patients.
Radiologically, 36.67% of patients had grade II Fisher’s classification of SAH, 30% had a grade III and 33.33% had a grade IV while the site of tSAH ranged between basal in 23.34%, convexity in 10% and diffuse type in 66.66% of patients. 16.67% of patients had associated EDH and 12% had SDH. Thirty percent of patients had an ICH, 16.67% had an IVH, 20% had hemorrhagic contusions, 23.33 % had depressed skull fractures and 6.67 % of patients developed post-traumatic hydrocephalus.
Forty percent were operated upon. 100% received cerebral dehydration and 50% received Nimodipine. The other 50% of patients who did not receive Nimodipine were the control group.
