Abstract

Anterior and nasal transposition of the inferior oblique muscle is one of the recent IO weakening procedure described by Stager at 2001.
In ANT IO, The muscle is reattached to the sclera with the posterior – temporal fibers attached 2 mm nasal and 2mm posterior to the nasal extent of the inferior rectus muscle insertion .The new insertion is not spread out , and its width was approximately 2mm to 3mm .
ANT IO convert the inferior oblique muscle from an extorter and elevator in adduction to an intorter and tonic depressor in adduction .
This procedure proved to have potent correcting effect on V pattern and inferior oblique muscle overaction with no significant postoperative pattern.
ANT IO have a limited effect in dealing with vertical deviation up to 18 ∆.
ANT IO have better postoperative outcome in DVD cases <15∆ than those with DVD ≥ 15∆ , but 100% curable rate for DVD is not exist.
