Abstract The risk of prostate cancer in Egypt is much lower than values used in many other countries. It seems that a PSA cutoff value of 4 ng/mL may be applied to the Egyptian population. We recommend prostate biopsy when the PSA level is greater than 4 ng/ml. or presence of abnormal digital rectal examination, even in the absence of abnormal transrectal ultrasonography findings. Although use of PSAD and PSADTZ can enhances the specificity of serum PSA for prostate cancer detection and decrease the number of unnecessary biopsies in males with PSA between 4.0 and 10 ng/ml., a number of prostate carcinomas may be overlooked. The rate of prostate adenocarcinoma in patients with PSA level between 2.5 and <4ng/ml and have normal DRE is very rare. And there is no indication to decrease the cutoff point of PSA in Egyptian patients. Regardless of other findings, all patients with a PSA value more than 10 ng/ml require biopsy because of the high likelihood of cancer. All patients with abnormal DRE or TRUS results still require biopsy. |
