Abstract

Hypospadias, is a congenital malformation of the male urethra, it is a common birth defect in humans and is reported to be increasing in incidence.
Hypospadias is the most common anomaly in male genital tract with approximately 0.8–8.2 per 1000 male newborns.
Many classifications for hypospadias were made in a trial to select the best operation for each type, all of them depend on the site of the meatus especially after correction of the chordae.
Surgery to repair hypospadias has a history of more than 150 years. To date more than 300 surgical procedures with accompanying variation have been proposed for the repair of hypospadias. Such repair should be simple, easily learned, applicable to the majority of cases, completed in a single stage and resulting in pleasing cosmetic result with a low complication rate.
Whenever possible, a single-stage repair of hypospadias is desirable. The Snodgrass (TIP) repair is widely used for distal hypospadias.
Snodgrass urethroplasty for distal hypospadias is a common procedure and accepted worldwide. It is a landmark in the history of hypospadias repair. It has lowest reported incidence of urethral fistula and meatal stenosis but at the same time gives circumcised penis
In this thesis 40 patients with primary hypospadias have been subjected to TIP repair as a definite treatment for hypospadias repair the cases were divided into two groups : group 1 was twenty cases who had been operated with doing spongioplasty only without using dartos fascia flap , group2 was also twenty cases who had been operated with doing spongioplasty plus using dartos fascia flap ,the mean age for each group was (3.32years ± 1.87) and (5 years ± 2.05) respectively , the mean operative time for each group was( 46.25min. ± 7.58) and (67.5 min.± 15.76) with significant stastistical difference (p value =<0.001*) with about 20minutes difference between two groups ,the incidence of intraoperative bleeding was 10% and 20% respectively ,the incidence of urethrocutaneous fistula was 10% and 15% respectively, the incidence of meatal stenosis was 10% and 5% respectively .
In this study although there is no statistically significant difference between two groups except the operative time , but this study suggests that the use of dartos flap in the repair of primary cases of hypospadias in TIP repair gives no significant advantages as regarding prevention of urethra-cutaneous fistula, meatal stenosis, on the opposite hand it consumes approximately 20 minutes in dissection of the flap and its fixation so the present study suggests that spongioplasty only in TIP repair without using daros flap is relatively safe ,rapid technique in primary cases of hypospadias .
