Abstract

Patient satisfaction is an evaluation of quality of care and plays a big role in clinical mental health care rendered to the patients.Patients who are satisfied with quality of care are more likely to follow treatment and, consequently, to have better health outcomes.Patient satisfaction about quality of care has been found to be an important factor related to inpatient aggression which can be triggered by factors related to the environment and ward staff. An association between staff experiences of violence and patient satisfaction about quality of care has been found. Thus, we can say that patients’ dissatisfaction about quality of care can lead to committing aggressive behavior. The aim of the study was to:Assess the relationship between patients’ satisfaction about quality of care and aggressive behavior in inpatient psychiatric hospitals.Sample: a convenience sample of 100 patients was chosen consisting of all available patients from two psychiatric hospitals who meet the inclusion criteria their age ranging between 20-61 years old.Design: A descriptive co relational design was utilized in conduction of this study.Setting: Meet Khalaf Psychiatric Hospital and Tanta Psychiatric Hospital.The following instruments were used to fulfill the aim of the study:Tool (1) Structured interview questionnaire:The structured interview questionnaire sheet was developed by the researcher for assessing socio-demographic and clinical characteristics of patients. It was divided into two parts:Socio-demographic characteristics of the patients as age,sex, educational level etc.Clinical characteristics of the patients as diagnosis, way of admission, period of disease etc.Tool (2) Quality of psychiatric care questionnaire:-This tool was devolved by Gigantescol, Morosinil and Bazzoni(2002) for measuring psychiatric inpatient satisfaction with quality of care. It was translated by the researcher into Arabic and was tested again for it’s content validity and reliability.Tool (3) The Aggression and Social Dysfunction Scale (SDAS-9)This tool was developed by Bech (1996) for measuring inpatient aggression. It was translated by the researcher into Arabic and was tested again for it’s content validity and reliability.
The main findings of the study revealed the following
1. The mean age of the studied sample was 34.98±11.88.2. More than half of the studied sample have low satisfaction about the quality of care.3. About one third of the studied sample use physical acts of aggression.4. There was a highly statistically significant negative correlation between patient satisfaction about quality of care and their forms of aggression.5. There was no statistically significant difference between
socio-demographic characteristics, e.g. (age, sex, educational level, job, income and marital status) and patient’s satisfaction about quality of care and also aggression.6. There was no statistically significant difference between clinical characteristics, e.g. (diagnosis, way of admission,hospital name, period of disease, hospital admission times for the same disease and way of treatment) and patient’s satisfaction about quality of care and also aggression.7. There was no statistically significant difference between smoking and patient’s satisfaction about quality of care.
8. There was statistically significant relation between smoking and forms of aggression.Based on the findings of the present study, the following recommendations are suggested:1- Educational program for psychiatric nurses about:A- Effective therapeutic communication with psychiatric patients.B- Medication and its side effects and discharge plan.C- Activity therapy and the importance of its application to psychiatric patients in improvement of patient’s symptoms and reduction of aggression.D- Effective ways of managing aggression.2- The ministry of health should give a great attention to:A- Psychiatric hospital environment and services as: Increase number of beds.Developing seclusion room.• Special room for food and a special place for activity therapy.Special place for visits and a garden for recreation.B- To give educational and training program for doctors regarding different forms of psychotherapy and the importance of its application.Suggested further study.Future research could assess the role of the care providers’ perspectives and how they contribute to the patient’s appraisal of satisfaction with the quality of psychiatric care.
