Abstract

This study was conducted on 64 congenital hypothyroid children who are registerd in the endocrinology clinic, children’s hospital Ain• shams University. Their ages ranged from one month to 25 years.They were divided into 4 groups according to thyroid ultrasonography (normal volume,goiter,hypoplasia and athyreosis).
All cases were subjected to * medical history taking • physical examination (including anthropometric measurements and pubertal staging) and IQ assessment.
*X-ray of left hand and wristwas done to determine the bone age,
* Estimation of serum T4 and TSH
*Thyroid US and scintigraphy using Tc99m pertechnetate
There was a female predominance in CH. No significant differences were found in age at diagnonsis, IQ level and growth parameters SDS among the 4 groups, but there was a highly significant difference as regards initialserum TSH concentration and thyroid gland volume between the different groups.
There was a negative non significant correlation between IQ level and dose of l.thyroxine.
Also, there was a negative non significant correlation between growth parameters and dose of L.thyroxine used in the studied patients. from this study, we can conclude:
* Early detection and treatment of CH are essential for normal growth and intellectualdevelopment of the affected children.
• The most common etiology of CH is thyroid dysgenesis and dyshormonogenesis is the second commonest cause of CH.
• Thyroid US is useful for detection of hypoplastic, goitrous and normal thyroid gland but it was not very sensitive in the detection of ectopic glands and therefore athyreosis can not be confirmed with thyroid US alone.
• There was no effect of the etiology of CH on both the physical and mentaldevelopment of hypothyroid children.
