Abstract

Erectile dysfunction (ED) is one of the most common sexual problems that affect men in different decades of life.
It is defined as a persistent inability to attain and maintain an erection sufficient to permit satisfactory sexual performance.
Psoriasis is a common chronic inflammatory, immune-mediated disease that predominantly affects the skin and joints. chronic inflammatory systemic disease, such as psoriasis, are a part of spectrum of chronic disease in which inflammation plays an important role in pathogenesis; central to this pathogenesis is an imbalance in inflammatory cytokines.
Atherosclerosis is a progressive disease characterized by the accumulation of lipids and fibrous elements in the large arteries. It is a main cause of erectile dysfunction, heart disease, stroke, and the underlying cause of approximately half of all deaths in westernized countries.
Common pathogenic features shared between psoriasis and atherosclerosis include immunological processes, inflammatory cytokines profiles, and the presence of inflammatory markers both locally and systemically.
In this case-control study, 50 psoriatic male patients and 50 age and gender-matched controls were enrolled. Psoriatic patients were subjected to full history taking, examination, PASI scoring, laboratory tests in the form of fasting blood glucose and lipid profile to identify patients with metabolic syndrome (MS) and measurement of both Ankle Brachial Index(ABI) and Carotid Intima Media Thickness (CIMT) to detect sub clinical atherosclerosis. Both patients and controls were assessed as regard the erectile state using the five item version of International Index of Erectile Function (IIEF-5).
We found that patients with psoriasis had a statistically significant difference as regard both ED prevalence and severity and that their ED is associated with decreased ABI and thickened CIMT indicating the arteriogenic nature of their ED due to atherosclerosis.
However , our data as regard PASI score and presence of MS showed no statistically significant association with IIEF score , possibly due to small sample size.
Conclusion:
In conclusion, our study showed increased ED prevalence in psoriatic patients as compared to tha age matched controls , particularly those affected with atherosclerosis even if sub clinical as evidenced by decreased ABI and increased CIMT of patients. However, we couldn’t find an association between the erectile state and PASI score or the presence of MS.
Our data emphasizes the need for early and proper treatment of the inflammatory process in psoriasis patients as well as better monitoring of traditional atherosclerotic risk factors to reduce cardiovascular mortality and morbidity and considering ED an early alarm as regard these complications.
