Abstract

The most common form of diabetes is type 2diabetes. Foot disease is a common complication of diabetes that can have tragic consequences.
Abnormal plantar pressures are considered to play a major role in the pathologies of neuropathic ulcers in the diabetic foot. .
Aim: This study was conducted to objectively evaluate the possible relation between plantar pressure and glycemic control in diabetic patients with and without neuropathy.

Our patients included 50 type2 diabetes mellitus, 20 of them had diabetic neuropathy their age group from 18 to 60 years
Type 2 diabetic patients with past history of amputation; associated rheumatologic or any other musculoskeletal disease and, associated foot wounds or ulcers were excluded.
We compared between the three groups (control, diabetic with neuropathy and without neuropathy, Patients were sub-grouped according to diabetes duration and compared with plantar pressure values.
In this study we measured the plantar pressure of different areas of the foot (medial heel (MH),lateral heel (LH) ,mid foot (MF), metatarsal(M) 1,2,3,4,5,toe (T)1,2,3) in our 3 groups in static and dynamic sessions.
Results: In our study, the static and dynamic plantar pressures in both right and left foot were significantly higher in diabetic with neuropathy group than diabetic without neuropathy group compared with control group in all the measured areas
Our results showed that deformities and diabetic duration are not statistically significant
In our study, there were significance in comparison between controls and diabetic without neuropathy group as regard plantar pressure especially in metatarsals.
Patients were further sub-grouped according to diabetes duration where the number (n) of patients with diabetes duration less than10 years. Age and HbA1c and BMI were compared in both subgroups and there was a statistically significant regarding age and HbA1c% while BMI showed no significant difference.
We found that diabetic patients with neuropathy have elevated peak plantar pressure more than diabetic without neuropathy compared with control group.
Risk factors like uncontrolled diabetes, foot deformity, and neuropathy may explain impaired distribution of plantar pressure in subjects with diabetes.
