Abstract

Background: Cesarean section is one of the most frequently performed surgical procedures by the obstetricians. After 13 years of consecutive increases, the total U.S cesarean delivery rate reached a high of 32.9% of all births in 2009. The total cesarean delivery rate declined in 2010 to 32.8% and was stable for 2011 and 2012.
Aim of the Work: The aim of this study is to determine the efficacy of preoperative vaginal cleansing with an antiseptic solution (10%povidone Iodine) in reducing post cesarean endometritis.
Methodology: This study was carried out at Ain Shams Maternity Hospital. Study type: It is a randomized case control prospective study. Study design: Recruited women were distributed into two groups: Study group (group Ι): 153 women who underwent cesarean sections received preoperative vaginal cleansing with 10%povidone Iodine. Control group (group Π): 153 women who underwent cesarean sections would not receive preoperative vaginal cleansing even with tap water. All patients received preoperative prophylactic intravenous antibiotics (third generation cephalosporin) with induction of anesthesia.
Results: Endometritis is a complication that occur after labour and more common after cesarean section than normal vaginal delivery. Endometritis is an infection of endometrium (lining of the uterus) with a characteristic triad (fever 38oC and more, uterine tenderness and offensive vaginal discharge) which need intravenous broad spectrum antibiotics. This infection is polymicrobial that move up from vaginal tract and cervix to infect endometrium.
Conclusion: This case control randomized study show that preoperative vaginal cleansing with 10% povidone iodine may decreased the risk of postoperative endometritis in women with elective cesarean section.
Recommendations: More studies with larger size are needed to know the effect of vaginal preparation with povidone iodine and other antiseptic solution before cesarean section in decreasing the risk of post-operative endometritis and other postoperative infections.
