Abstract

Background:Obesity is a major public health problem with an increasing prevalence. Although open heart surgeries are now performed with low morbidity and mortality rates, obesity is still assumed to be an important risk factor for morbidity and mortality at these surgeries. Aim: the aim of this study was to investigate the relationship between obesity and selected open heart surgical outcome among obese versus matched non obese open heart surgery patients at Cairo University Hospitals during a period of six months. Research questions: Q1.What is the frequency of obesity among open heart surgery patients at Cairo University Hospitals during a period of six months?; Q2.What is the open heart surgical outcome of obese versus matched non obese patients at Cairo University Hospitals during a period of six months?;Q3.What is the relationship between obesity and selected open heart surgical outcome among open heart surgery patients at Cairo University Hospitals ?Sample:A purposive sample of62 adult male & female open heart surgery patients who were admitted to ICU over a period of six months divided into two groups; (31)obese with BMI of ≥ 30kg/m2 and (31) non obese with BMI of (18.5-24.9)kg/m2. Setting: The Cardio-Thoracic Intensive Care Units affiliated to Cairo University Hospitals. Tools: Two tools were utilized to collect data pertinent to the study: Socio demographic and medical data tool and Post-open heart surgical outcomes assessment tool. Results: The current study revealed that, (27.6 %, 42.7%, 29.7 %) of open heart surgery patients were within normal BMI, overweight and obese respectively.The majority of patients undergone open heart surgery were males 74% of obese group and 77.4% of non obese group and the most common open heart surgical procedure was coronary artery bypass graft (CABG )74.2% of both groups. Atrial fibrillation was a common complication among obese and nonobese patient (22.6%&12.9) respectively . No significant statistical correlation was put into evidence between obesity and post open heart surgical outcome except for Length of hospital stay.Conclusion:Obese patients did not experience significantly greater morbidity and mortality compared with non obese patients except for Length of hospital stay(p value=0.04). Obesity alone was not found to be associated with hospital morbidity or mortality after open heart surgeries.Recommendations: based on finding of thisstudy the following are recommended: Establishment of specialized hospital documentation system and data base for open heart surgeries at Kasr Al-Ainy Hospitals; Regular follow up for obese patients to evaluate their health conditions and to detect any anticipated complications as early as possible and Establishment of weight loss program for obese patients preoperatively.
