Abstract One of the most difficult situations that faces any phaco surgeon is intraoperative miosis and poorly dilated cases. Regardless of surgical technique, adequate mydriasis is essential for all stages of cataract surgery. Pupil contraction during phacoemulsification or the irrigation/aspiration phase increases the chances of iris damage, incomplete cortical material removal, posterior capsule rupture, vitreous loss and difficulty with intraocular lens (IOL) implantation into the capsular bag. This study included 90 eyes of 82 patients. All underwent uneventful phacoemulsification between December 2014 till December 2015. All patients were evaluated preoperatively for their BCVA, IOP, CCT, Endothelial cell count, CMT, Pupil diameter, pulse rate, blood pressure. We reached an understanding that intracameral epinephrine even in higher concentrations is effective in pupillary dilataion especially in cases with poorly dilated cases by usual topical mydriatics. It is also safe on cardiovascular system, cornea and macula and doesn’t affect visual outcome when it is used, but even it decrease complications by proper dilatation. |
