Abstract

Background: GER is the passage of gastric contents into the esophagus and is referred to as GER disease (GERD) when GER causes troublesome symptoms and/or complications. GERD in infants and children is sometimes difficult to diagnose and even more difficult to treat. In neonates relatively few GER episodes causes esoghageal acidification to PH <4. Aim: To study gastroesophageal reflux disease in neonates using combined multichannel esophageal impedance and PH metry pre and post proton pump inhibitor therapy. Patients: this study included 20 neonates (preterm and fullterm) with symptoms and signs suggestive of GERD. Results: Proton pump inhibitors therapy significantly reduced the number of acidic GER but doesn’t affect weakly acidic and non-acidic GER which constitutes the main load of GER episodes in neonates. Concerning the refluxate in the proximal esophagus, it was demonstrated that weakly acidic reflux is the one that reaches proximal esophagus hence this could be the reflux causing the troublesome symptoms and signs especially that it was not relieved by proton pump inhibitors suggesting a relation between this type of reflux and patient’s signs. Conclusion: The study is to be done on a larger scale for better evaluation of the effect of proton pump inhibitors treatment on neonatal reflux using multichannel intraluminal impedance abd PH metry. The use of multichannel esophageal impedance and PH monitoring is limited for determining the degree of pathological reflux in neonates because there are still limited studies on normal values in neonatal population.
