Abstract

Till now there are no clear protocol and recommendations for MRI in diagnosing FAI and this more evident as regard chondral lesions, also there was no study comparing MRI finding with hip arthroscopic finding as regard age groups so we conducted our study focusing on adolescent age group suffering from FAI. Different scanning protocols have been developed such as cMRI, dMRA and iMRA. Several studies compared the accuracy of these different protocols but there is a debate about whether introduction of contrast material increases the accuracy of cMRI or not. Introduction of contrast material may be done directly by intra-articular injection into the joint as in dMRA or indirectly by intravenous injection as in iMRA. We reviewed the data of 48 adolescents with persistent hip pain associated with FAI and were treated by hip arthroscopy after failure of conservative treatment. MRI was done for all patients before doing hip arthroscopy. We compared MRI findings with surgical finding to assess the effectiveness of MRI in identifying acetabularlabral and chondral lesions. Conclusions : The findings of our study confirm the requirement for a high clinical index of suspicion to diagnose symptomatic chondral and labral pathologies in adolescents because the accuracy of both cMRI and dMRAweren’t high. Direct MRA is more accurate than cMRI in detecting labral and chondral lesions in adolescents. The diagnostic test accuracy was superior in detection of labral lesions when compared with detection of chondral lesions in both cMRI and dMRA.
