Abstract

Acute subarachnoid hemorrhage (SAH) is a medical emergency, which can lead to a high rate of mortality and many severe complications. It is critical to make rapid radiological evaluation of ruptured intracranial aneurysms to help determine appropriate surgical treatment of SAH.
Digital subtraction angiography (DSA) has been considered as the “gold standard” for detection and therapeutic decision-making of intracranial aneurysms. However, it is an invasive and time-consuming technique with a reported 0.5% risk of neurological complications.
As a result of development of CT scanners and workstation, three-dimensional computed tomographic angiography (3D CTA) with helical scanning has been widely applied in neurovascular imaging. The rapid innovation of multi-detector row CT has provided increased spatial resolution and decreased scanning time, which makes it become a useful alternative imaging method for the evaluation of patients with suspected intracranial aneurysms. CTA had relatively high sensitivity and specificity in the diagnosis of cerebral aneurysms, it has disadvantage in detection of aneurysms with small-size and near the skull base due to the influence of overprojecting bone structures. Subtracted 3D CTA, which allows bone-free visualisation of aneurysms, has been developed and compared favorably with DSA for the detection and characterisation of intracranial aneurysms.
