Abstract

Temporal lobe epilepsy (TLE) is the most frequent cause of focal and refractory seizures. Patients with temporal lobe epilepsy commonly demonstrate gliosis and loss of neurons within the hippocampus and the dentate nucleus. These changes designated Hippocampal sclerosis. Neuroimaging evaluation of the brain and accurate lateralization of the epileptogenic focus are proving to be helpful and important for a successful temporal lobe epilepsy operation. MR imaging remains one of the the fundamental neuroimaging technique for identifying a lesion that could be responsible for the epilepsy due to the high tissue resolution and the multiplanner ability . Lateralization of the affected hemisphere in TLE patients, particularly in case of no pathological findings in high resolution MRI is still a challenge. Magnetic resonance spectroscopy (MRS) has the advantage to measure invivo the cerebral metabolites non-invasively on the same clinical MR
systems, allowing detection of the abnormalities before the appearance of
structural changes. The aim of this study was to evaluate the diagnostic role of magnetic resonance imaging(MRI ) and magnetic resonance spectroscopy(MRS) in diagnosis of temporal lobe epilepsy .In this study , we were able to recruit 100 cases that referred to the radiodiagnosis department at Tanta University hospital from neuropsychatry department ,neurosurgery department and outpatient’s clinics.
