Abstract Umbilical hernia repair, a common day-surgery procedure in children, is associated with considerable postoperative discomfort. A rectus sheath block can provide effective pain relief for umbilical or other midline surgical incisions.This regional anesthetic technique has become increasingly popular and is used to provide analgesia for umbilical and epigastric hernia repair,laparoscopic surgery, pyloromyotomy and other small midline incisions. The use of ultrasound as an aid for accurate placement of local anesthetic is gaining in popularity for regional anesthesia. Caudal anesthesia has been used for many years and is the easiest and safest approach to the epidural space. This study was carried out in Tanta university hospitals in pediatric surgery department on 60 patients of both sexes aged from 1 to 6 years old with ASA physical status I and II for six months from January 2015 to June 2015 to compare postoperative analgesic effect of U/S RSB and CB in patients scheduled for elective umbilical hernia repair. Exclusion criteria included: Parents refusal, upper air way infection 2 weeks ago or less, \ASA physical status 3 or more, patient aged above 6 years or below 1year,fever, allergy to bupivacaine, bleeding and coagulation disorders and patient with neurological problems; specially affecting lower limb.
