Abstract Background Appropriate embryonic development, coincident with a receptive endometrium, which occurs during the mid-luteal phase of the menstrual cycle, is very important for successful embryo implantation. The reduced endometrium blood flow may impair the growth of the glandular epithelium, resulting in decreased expression of endometrial vascular endothelial growth factor. Methods Randomized controlled clinical trial was conducted at Ain Shams University Maternity Hospital at infertility outpatient clinic comparing the pregnancy rates with oral sildenafil citrate 25 mg tab/6hours versus placebo in females undergoing induction of ovulations by clomiphene citrate 100 mg/ day in polycystic ovary syndrome women among 210 infertile patients for three consecutive cycles then the females who didn’t get pregnant in both groups have been crossed over after one menstrual cycle wash . Results This study shows statistically significant results for endometrial thickness and pattern, subendometrial blood flow and just statistically significant result for pregnancy rate with p value < 0,049 in the first step and < 0,047in the 2nd step which means that using of sildenafil citrate is effective in improving endometrial thickness and pattern, subendometrial blood flow and has weak effect on pregnancy rate Conclusion This case control randomized study show that using sildenafil citrate in patients with PCO syndrome undergoing induction of ovulation increases the clinical pregnancy rate and the endometrial thickness and improves the endometrial pattern with improving the endometrial Doppler indices . |
