Abstract

Hepatitis C virus over years can develop into fibrosis cirrhosis then to hepatocellular carcinoma, AFP is the most common marker for HCC detection with Ultrasonography. AFP is limited as a marker owing to low sensitivity and US depend on operator’s experience. NMP-52 is released from dead cell nuclei in liver disease. NMP investigated as a marker of organ damage.so combination between AFP and NMP-52 enhance discrimination of HCC patients from those with non-malignant liver cirrhosis. HCC is the 3rd reason of cancer related death worldwide. Egypt has the highest prevalence of HCC in the world. HCV and HBV are the most common cause of HCC.Serum markers for HCC are altenative to imaging and Liver Biopsy for both patients and clinicians. AFP is the most common marker but owing to its low sensitivity its use is limited. Enhancing the HCC diagnostic ability its combined with nuclear matrix protein (NMP-52). Serum samples from healthy individuals and patients with cirrhosis and HCC were separated by SDS-PAGE. The separated proteins were electrophoretically transferred to nitrocellulose paper by Western blot technique that is used to identify NMP antigen using NMP mono-specific antibody. NMP-52 antigen level was quantified in different stages of liver cirrhosis and HCC by ELISA technique. An intense sharp band at 52 kDa was detected in cirrhosis and HCC patient’s sera but there is no reaction with sera of healthy controls. The detection rate of NMP-52 increase with disease progression as its higher in HCC patients than cirrhotic patients. Combination between NMP and AFP increase the accuracy of HCC detection. HCC patients were associated with higher levels of NMP-52 than patients with cirrhosis. There was significant weak correlation between AFP and NMP-52 in HCC patients. the AFP/NMP-52 has high diagnostic power. Thus, we can recommend the use of AFP/NMP-52 ratio to not only give serious data in HCC detection and therapeutic follow up but also to monitoring the disease.
