Abstract

Critically ill patients are at risk of resistant infection for several reasons, including, abnormal immune systems, high use of antibiotics, invasive procedures, decreased resistance to infection, reduced nutrition, and prevalence of resistant infections in intensive therapy unit. Assessment standard precautions are designed to protect healthcare professionals and patients from exposure to potentially infected blood and body fluids by applying the fundamental principles of infection prevention. Therefore the Aim of this study was to identify Assessment of standard infection control precautions among health care team in intensive care units at Assiut university hospital Main. Design: descriptive research design was utilized in this study. Setting: The study was carried out in intensive care units at Assiut university main hospital. Subjects: The study comprised assessment of 295 staff from intensive care team in Assiut University Main Hospital. A total of (31) physicians, (202) nurses and (62) workers participated. Variations in the number of questions between physicians, nurses, and workers personnel were related to their different roles in practice in the ICUs. Tools: two tools were utilized to collect data pertinent to the study which are: Tool I: Assessment of the knowledge of health team in intensive care units regarding standard infection control precautions. This tool consist of structured multiple choice pre- test questionnaire sheets. This tool was developed by researcher and composed of 7 main parts as socio- demographic data, training methods about infection control precautions, vaccination against viral hepatitis (B), observation and evaluation about practice of medical hand washing, exposure to injury during work, availability of supplies and resources, & personal protective equipment available in the department for health team. Tool II: Performance assessment tool: (An observational checklist sheet). This tool composed of 13 main parts include done or not done (complete, incomplete) as Performance related to hand washing, personal protective equipment, insertion intravenous cannulas, intravenous lines (change/insertion), Suction care, insertion and removal of urinary catheter, intubation patient care, dressing wound, needle &sharps, environment control, linen, total score , and level of health team performance. Methods: the study was conducted throughout three main phases, preparatory phase, implementation phase and evaluation phase. The main results, Results: The current study revealed that there was significant statistical difference in the number of nurses, physicians, and workers as regard personal protective equipment and medical hand hygiene, causes of acupuncture needle, insertion and change of intravenous lines, intubation patient care, insertion of intravenous cannulas, urinary catheter, suction care, and environment control. The majority of nurses and physicians are attending training courses for infection control but majority of workers aren’t attending training courses for infection control. However, the vast majority of nurses and workers, more than half of the physicians had adequate performance level with a mean total performance scores of (161.76±8.71) for nurses, (146.83±12.16) for physicians, and (157.48±15.73) for workers. Conclusion: Based on findings of the current study, it can be concluded that satisfactory performance level regarding infection control standard precautions. Recommendation: updating knowledge and performance of ICU nurses through continuing in-service educational programs; emphasizing the importance of following the latest evidence-based practices of infection control in continuing education / training programs; strict observation of nurses’ performance/ utilization of infection control standard precautions and correction of poor practices by the infection control team are required; and providing training programs for newly joined ICU nurses about infection control standard precautions and at regular intervals.
