Association Between Mean Platelet Volume And Unexplained
Recurrent Miscarriage
Abstract
Introduction: According to the Royal College of Obstetricians and Gynaecologists (RCOG), a miscarriage can be defined as the spontaneous loss of a pregnancy before the fetus has reached viability at 24 weeks. This includes all pregnancy losses from the time of conception until 23 completed weeks of gestation. RPL is defined as three or more consecutive miscarriages. The World Health Organization recommends that in developing countries, where gestation is often uncertain, a birth weight of 500 g should be used to define viability. The American Society for Reproductive Medicine defines RPL as two or more failed pregnancies, which have been documented by either ultrasound or histopathological examination. They suggest that some investigation must be done after each miscarriage, with a thorough evaluation after three or more losses.
Aim of the work: The aim of this study is to assess the association between mean platelet volume (MPV) and recurrent miscarriage.
Patients and Methods: Study design: A Prospective observational case control study. Setting: This study was conducted at the recurrent miscarriage clinic in Ain Shams Maternity University Hospital after approval of the Ethical Research committee during the period from April 2016 till January 2017.
Results: In this study there was no statistical significant difference between recurrent miscarriage groups and control group in the haematological parameters of complete blood count including, PDW, Platelet distribution width (%); RDW, Red cell distribution width (%); PCT (x103/mm3): Platelet count Hb, Hemoglobin (g/dl); Htc%, hematocrit; wbc (x103/mm3), white blood cell. While MPV (fl), there is statistically significant difference in the RPL group compared to control group. This study detected significantly higher MPV values in patients with recurrent miscarriage. Our results support the thesis that increased MPV leads to thrombosis, which in turn leads to micro-emboli in the uteroplacental circulation, placental inflammation and insufficiency and consequently recurrent miscarriage.
