Abstract

Background: In this study 50 pregnant women were rolled in a prospective and comparative study from both the attendants outpatient clinic wards of Obstetrics and Gynecology Department in Ain Shams University Hospital. Aim of the work: This study aims to examine the relation between Maternal Serum insulin and leptin levels in the first trimesters in patients with unexplained recurrent pregnancy loss. Patients and Methods: After approval by the Hospital Ethics Committee, and written informed patient consent with an explanation regarding the purpose, methods, effects and complications. We studied 72 pregnant women in a prospective and comparative study. Results: In the current study as regarding the prediction of early pregnancy loss by estimation of insulin level, it was found that its cutoff point was > 26.52, its sensitivity was 60.0, its specificity was 100.0, positive predictive value 100.0, negative predictive value 71.4 and its accuracy was 0.811. Regarding leptin its cutoff point was >14.06, its sensitivity was 92.0, its specificity was 96.0, positive predictive value 95.8, negative predictive value 92.3 and its accuracy was 0.982 and in comparison between both the difference between areas (area under curve) was 0.171 with standard error =0.064 and significant P-value = 0.007 so, in the current study the leptin level is more accurate than insulin in prediction of early pregnancy loss. Conclusion: According to the results of this study it was found that serum leptin and insulin hormones levels were higher in cases with history of previous early RPL and their levels were higher in cases aborted from them during the study. Also, leptin was more accurate in prediction of early RPL than insulin. Recommendations: from this study, we recommend that estimation of the serum levels of leptin and insulin hormones should be used in cases of RPL as a predictor of pregnancy continuation or not. Further studies are needed to confirm these results.
