Abstract

T
his retrospective study was conducted at clinical oncology and nuclear medicine,Ain Shams University, on 32 female patients with BC metastatic to the brain. The objective of this study was to evaluate prognostic and predictive factors in BC with BM.
Our study, The mean age at diagnosis of BM was 48.7years ±11.39.It ranges from 27 – 69 with median age 49.5 years,17 cases (53.13 %) were more than 48 years,15 cases (46.88 %) were less than or equal 48 years. The majority were postmenopause 59.38% and only 2 of all cases had positive family history (6.3). Grade 3 PS had the majority of cases (43.75%) then grade 2 PS (34.38%).The main histological type was IDC 81.25%,, tumor grade 2 consist 65.63%. HER 2 positive irrespective of hormonal status were seen in 50% of patients. The majority had manifestation of increased intracranial tension 68.7 %, Multiple BM had the majority of cases 68.8 %.Brain as the only metastatic Site was seen in 9 cases (28.1 %). At presentation of BM, both of only brain and brain with one organ had the majority of cases (40.6 %,34.3 % respectively).Primary treatment of BM was either surgery seen in three cases (9.4 %) also chemotherapy was seen in three cases(9.4 %) while radiotherapy had the majority of cases was seen in 26 cases(81.2 %). After a median follow-up time of 10 months after diagnosis of BM, all patients were dead. Time to BM (TTBM) is defined as the interval of time from BC diagnosis to the date of BM diagnosis. Median TTBM was 16 months (95% CI: 13.228 -18.772). PFS was defined as the interval of time from diagnostic BC metastatic brain till progression or death. The median PFS was 4.5 months. Regarding PFS, there were statistically significance relation found with Tumor subtype and mass excision as shorter PFS in TNBC with median 3 month p value 0.007 and longer PFS with Mass excision with median 31 month P- value 0.023 OS was defined as the interval of time from diagnostic BC metastatic brain till death. The median OS was 6.5 months. Regarding OS, There were statistically significance relation found with (ECOG PS at BM, mass excision Tumor subtype and number of distant met. at BM presentation) as longer OS were seen in ECOG PS 0 – 1 with median 12 months p value 0.037 and Mass excision with median 31 months p value 0.007, while shorter OS were seen in TNBC with median 4 months p value 0.045, and increase number of distant met. At BM presentation with P- value <0.001.
