Abstract

Background Hepatitis C Virus is both hepatotropic and lymphomatropic virus and is estimated to affect over than 180 million individuals worldwide. chronic hepatitis C infection is associated with several hematological manifestations such as monoclonal-gammopathy of uncertain significance (MGUS), mixed cryoglobulinemia (MC), and B – cell non – Hodgkin lymphoma (B – NHL). chronic HCV infection has been associated with B cell indolent lymphomas, especially marginal zone lymphoma (MZL), and aggressive lymphomas, mainly diffuse large B cell lymphoma (DLBCL) Objectives Our study aimed at to re-evaluate the role of HCV infection among patients with NHL, to study the relation between HCV infection in Non-Hodgkin lymphoma patients and the patients immunological state, and to study the characteristics of different histologic forms of Non-Hodgkin lymphoma in HCV positive patients in relation to different HCV genotypes and to viral load. Patients and Methods The present study was carried out on 100 patients with B-cell non Hodjkin lymphomas diagnosed by lymph node biopsy and immunohistochemistry, 50 patients with various malignant solid malignancies, and 50 patients with hepatic affection other than hepatitis C virus infection and 50 control healthy subjects. Patients were recruited from Ain-Shams University Hospital, Hematology Department and National Cancer Institute from the period of April 2013 to August 2015. All the patients were subjected to full clinical histoy, thorough clinical examination, laboratory investigations including CBC, ESR, CRP, liver profile, renal profile , Serum LDH, Serum β 2 microglobulin, Serum cryoglobulins, Serum Ig G, Ig M, Ig A, bone marrow aspirate and trephine biopsy, Anti HCV antibody detection by screening enzyme immunoassay (EIA), Detection and quantification of HCV RNA by (RT-PCR), Identification of HCV genotype by LiPA, abdominal ultrasound, CT neck, chest, abdomen and pelvis. Results The results of our study demonstrated that the most prevalent histpathologic subtype of B-NHL found among HCV +ve patients was MALT lymphoma (±SD 71.4 %). In the present study, the prevalence of HCV infection among B-NHL patients was found to be 43 % followed by 36 % in sloid tumors patients and all HCV + ve patients were having genotype 4 of HCV. Regarding the viral load, there was no significant difference between B-NHL and solid tumors , and viral load was positively correlated with S. LDH, S. β2 microglobulin and S. IgM levels, and inversely correlated with platelet count and prothrombin concentration in HCV +ve patients of B-NHL group and solid malignancies group. In our study, serum cryoglobulinemia was significantly identified in HCV +ve patients than in HCV –ve patients in both B-NHL group (74.4%) and control solid malignancies group (66.6%), with Ig G being the most predominant Ig subtype followed by IgM and IgA in B-NHL patient group and control solid malignancies group. Conclusion By reviewing the results of our study, we can conclude that the prevalence of HCVinfection among B-cell NHL Egyptian patients can be approximately estimated as 43% with altered immunological status of the sera of HCV +ve patients in the form of abnormal detection of serum cryoglobulins and elevation of serum immunoglobulins IgG & IgM as well as the elevation of serum levels of the biological markers LDH & beta 2 microglobulin.
