Abstract

Fever and neutropenia (FN) is a common complication in patients undergoing chemotherapy . Empiric treatment with intravenous broad-spectrum antibiotics in an inpatient setting is the mainstay of therapy for FN in pediatric patients. Invasive fungal infections are major causes of morbidity and mortality in those patients. Candida and Aspergillus species constitute the most common pathogens isolated in nosocomial bloodstream infection.
Aim of the work
Determine the association between clinical, laboratory and radiological parameters in pediatric oncology patients with febrile neutropenia in relation to microbiological results and serum galactomannan levels.
Patient and Methods
A cross-sectional study included sixty pediatric cancer patients with febrile neutropenia attending the Ain Shams University, Pediatric Hospital, Oncology Clinic. All patients underwent revision of clinical, laboratory and radiological data with testing for galactomannan results.Probable and possible invasive fungal infections were classified based on the EORTC/MSG definition 2008.
Results
The results of the study show that there were high significant between fungal infection and prolonged fever and hospital stay in patients undergoing chemotherapy to treat cancer.
Conclusion
We concluded that invasive Aspergillosis is a common disease in pediatric patients with febrile neutropenia. It was correlated with the severity of neutropenia and had a significant impact on the duration of hospital admission. Cut off level of 0.5 for galactomannan optical density needs to be translated cautiously in the setting of antifungal and pipercillin/tazobactam use.Key words: fungal infection, febrile neutropenia, serum galactamann test.
