Abstract

In the attempt to optimize the balance between the risk of local recurrence and the cosmetic outcomes in breast surgery, new surgical procedures, so-called oncoplastic techniques, have been introduced in recent years. The term oncoplastic surgery refers to surgery on the basis of oncological principles during which the techniques of plastic surgery are used, mostly for reconstructive and cosmetic reasons. The advantage of the oncoplastic surgery for breast cancer is the possibility of performing a wider excision of the tumour with a good cosmetic result. Careful patient selection and preoperative planning are key components for the success of any oncoplastic operation for breast cancer. Accurate preoperative evaluation of the clinical and biological features of the tumor as well as of the morphological aspects of the breast allow the surgeon to make a decision if a radical or conservative approach is preferable and select the most effective surgical technique. Benelli, in 1990, proposed a new periareolar mammoplasty, a round-block technique applied in several types of breast surgery such as ptosis or hypertrophy, and hypotrophy and tumor excision. In tumor excision, this technique produced a discreet scar and a more regular breast contour. De-epithelialization is fundamental in performing this technique; this procedure was introduced earlier by plastic surgeons when performing reduction mammoplasty. Round-block mammoplasty is reported as an alternative method in mammary reductions. It was modified and presented as a good surgical choice for all quadrant-located breast cancer in small- to medium sized breasts with good aesthetics results.
