Abstract

Intussusception is a common pediatric surgical emergency in Egypt. Diagnosis of intussusception usually based on the clinical history and is confirmed by clinical examination; however, a high index of suspicion coupled with proper radiological studies maybe necessary for diagnosis of atypical cases. Management of intussusception is initially non-surgical. Surgery is advised only when non-surgical therapy has failed or is contraindicated.
Our study aim are evaluate incidence, etiology and possible lines of management of recurrent cases after hydrostatic reduction.
Results we found in this study was helpful to put guideline and basic data to our work in pediatric surgery department in Assiut university hospital it was as the following:
- Total number of cases was treated by hydrostatic reduction as first line of treatment 140 patients.
- Sex of patients was 90 (64.3%) males and 50 (35.7%) females at ratio 1.8:1.
- Ages of patients were 81 (57.9%) younger than 1 year, 24 (17.1%) from 1-2 years, 30 (21.4%) from 2-5 years, 5 (3.6%) older than 5 years, at mean 1.7 year.
- Main clinical presentation was colicky abdominal pain presented at (79.3%) of patients and (30%) of patients presented with classical triad (abdominal pain, bleeding per rectum and vomiting).
- Majority of patients (44.3%) presented to hospital from 24 to 48 hours from starting symptoms, while majority of operated patients were presented to hospital after 48 hours (24.3%).
- Patients with idiopathic cause of intussusception were (85%), pathological lead point was found in 15 patients (10.7%) of primary intussusception cases.
- Total number of cases with recurrent intussusception was 20 patients, pathological lead point was found in 5 patients (25%), 4 of them older than 5 years.
- Total number of hydrostatic reduction trail at primary intussusception was 206 trails; successful rate was (80%).
- Total number of hydrostatic reduction at recurrent attack was 31 trails; successful rate of all recurrent episodes were (80%).
