Abstract

Introduction: Type 1 diabetes is a disorder that arises following the autoimmune destruction of insulin-producing pancreatic beta cells Type 1 diabetes. It is associated with other auto-immune disorder one of them is celiac disease which has ahigh prevelace rate among type 1 DM reaching (6%-16%).The most common used test is Anti-tissue Transglutaminase Antibody (anti-tTG),IgA class which is the primary test ordered to screen for celiac disease.
Aim of the study: Screening of coeliac disease among newly diagnosed type 1 diabetes mellitus chideren by assessement of Anti-tissue Transglutaminase Antibody (anti-tTG),IgA class.
Patient and method: This case control study was carried out on 50 child with newly diagnosed type1 diabetes mellitus attended to Pediatric Diabetes clinic ,Pediatric Hospital, Ain Shams University from August 2015 to December2015. Another group of 15 age and sex –matched healthy individual with no obvious medical disorder and not receiving any medication were enrolled as a control group.
Patients were subjected to detailed history, physical examination laboratory investigation. The laboratory investigation included initialHbA1c %,initial c-peptide,Thryroid function teste( T3,T4,TSH)total body IgA and anti-Ttg(IgA).
Results: The most common presentation of type 1 diabetes was polyurea (82%), polydipsia(84%),weight loss(74%)and diabetic ketoacidosise (58%). In the current study there is a high prevalence of thyroid disorder (hypothyrodisim 4%-6%) (hyperthyrodisim 4% -6%)among our type1 diabetic patient..prevelance of +ve TTg(IgA) Ab among our type1 diabetic patients was (12%) .Patients with +ve TTg(IgA) Ab were shorter than -ve TTg(IgA) Ab patients. HbA1c among our diabetic patients with +ve TTg(IgA) Ab and patients with -ve TTg(IgA) Ab was significantly elevated among +ve patients(p<0.05).The most common presenting symptoms of +ve TTg(IgA) Ab patients were chronic diarrhea and pale foal smelling stool. TTG (IgA)Ab can be used as screening tool for the occurrence of celiac disease among type1 diabetic patients with sensitivty70.59% and specificity 80%.
Conclusion: TTG(IgA) Ab levels are elevated in children and adolescents with type 1 diabetes compared with healthy controls. Prevelance rate of +ve TTG(IgA) Ab among type1 diabetic patients was 12%. TTG(IgA) Ab show no significant relation between it and other clinical parameters except symptoms of celiac and HbA1C which were high in those expressing seropositivity to ATTG antibody.Most common presentation of celiac disease was chronic diarrhea,abdominal pain,bloating and pale foal smelling stool.TTG(IgA) Ab test has sensitivity 70.59% and specificity 80%.so can be used as screening test for celiac disease among type1 DM.
