Abstract

This study was a prospective study aimed at determining the association of serum Lp(a) with the severity and complexity of coronary artery disease in patients with AVS (N=50) with the age mean of 60.76 years. All patients had different grades of AVS (18 patients with grade I, 14 had grade II and 18 had grade III).
Regarding the age and gender of the studied population, it was shown that of all patients, 16 patients were females with a percentage of 32 % and 34 were males with a percentage of 68 % of all patients.
As regard risk factors distribution: 58% (29 patients) of all studied population had high risky Lpa level and 42 %( 21 patients) had normal or non-risky levels. Also 50% (25 patients) of all patients were smokers, 26% (13 patients) were ex-smokers and 24% (12 patients) were non-smokers. Laboratory findings showed high total cholesterol (<200 mg/dl) and TLC (< 11,000 c/cmm) in 54% (27 patients) of all population versus 46% (23 patients) with normal or low levels. Hypertension and diabetes were found in 56% (28 patients) versus 44% (22 patients) who were non hypertensives or diabetics. Moreover, 76% (38 patients) had clear family history of IHD versus 24% (12 patients) with no family history of IHD.
The results of our study showed very highly significant increase in the number of diseased epicardial coronary vessels by CA in group I with high risky level of Lp(a) compared to group II with normal level with p-value (>0.001).
Also SYNTAX score (used for quantification of severity and complexity of coronary artery disease )was high (35.07 ±6.08 SD) in group I with high Lp(a) level compared to (14.76 ±4.5 SD) in group II with low Lp(a) with highly significant p-value (>0.01).
Among different grades of AVS, we observed mean age of 65.33 ys in grade III versus 56.22 in grade I along with significant presence of hypertension in 18 patients (with a percentage 100%) in grade III versus 5.56% in grade I, mean total cholesterol of 260.94 mg/dl in grade III versus 185.83 in grade I, ischemic ECG changes in 10 patients (55.57%) with grade III versus 3 patients (16.67%) with grade I and mean TLC of 14.61,000c/cmm in grade III versus 6.83 in grade I.
Also Lp(a) level was high in 15 patients with grade III AVS patients (with a percentage of 51.72%) versus 3 patients with grade I AVS (10.34%) and this was statistically significant (p-value > 0.05).
We concluded that serum Lp (a) level is considered an important predictor marker for severity of coronary artery disease in patients with aortic valve sclerosis.
