Abstract

Background: Melasma is an acquired symmetrical hypermelanosis that occurs in sun-exposed areas. Platelet-Rich Plasma (PRP) may serve as a source of different growth factors to reduce the pigmentation as in infraorbital dark circle and melasma.
Aim of the work: To evaluate the clinical efficacy of PRP as a new modality in melasma treatment and the value of intense pulsed light (IPL) as an activator for PRP.
Patient and Methods: Twenty female patients with facial melasma were treated with PRP combined with IPL (group 1) in one side of the face and PRP only (group 2) on the other side. All cases were assessed by Melasma area and severity index (MASI) score, modified MASI score, photos before and after treatment and patient and physician satisfaction scoring.
Results: There was no statistically significant difference with p-value >0.05 between two study groups regarding patient and physician satisfaction. Most results reported fair physician and patient satisfaction about both treatment regimen. The mean MASI score was (16.3±7.7) decreased after treatment to (10.9±6.3), the reduction in MASI score was 33.13%. Modified MASI score of group 1 decreased from 21.8 to 16.6 (23.85% improvement) and modified MASI score of group 2 decreased from 21 to 16.2 (22.86% improvement).
Conclusion: In conclusion, PRP is considered a moderately (fairly) effective treatment for melasma. We don’t advise to follow injection of PRP by IPL as it didn’t improve the results. However, further studies on larger scales are required.
