Abstract

Background: In DSM-5 OCD, depressive disorder, and bipolar disorder are presented as a possible different categories which is not preferred by many researchers who believe that, it would be better we develop an affective disorder spectrum including the 3 disorders. Objectives:The aim of this study is to compare among patient first diagnosed as Obsessive-compulsive disorder who will show manic mood switch and who will not during antidepressant treatment (SSRIs) within 18 months of follow up study. Participants and methods: this study was designed to be a cross sectional survey (6 months) study followed by prospective (cohort study) for one and a half year for first diagnosed patients coming to psychiatric department outpatient clinic in Mansoura University Hospital for 6 months duration (n=61 patients: 48 OCD; 13 OCD-BD). Results: 21.3% of first diagnosed OCD patients showed manic shift. Age of onset was younger in OCD than OCD-BD. Family history in OCD-BD showed more history of bipolarity in first degree relatives, antidepressant induced mania, early age of onset of depression. On clinical presentation it was noticed that, their was no difference in shape of obsession or compulsions. It was noted that OCD-BD needs more hospitalization, showed more suicidal thoughts , history of recurrent minor depressive disorders, brief major depressive disorders or atypical depression, better response to antidepressant than OCD without manic shift. Conclusion: OCD could be a first presentation for BD which needs more concern when treating first diagnosed OCD patient.
