Abstract

CTS is a common clinical problem, its diagnosis is based on clinical and EDS findings. US have enjoyed attention as a noninvasive method for diagnosis of CTS. The purpose of this study was to evaluate validity of US in the diagnosis of patients with clinically and EDS confirmed CTS. Measurements of CSA and AP diameter of MN were obtained in distal forearm region and in the carpal tunnel in patients and controls. Arithmetic mean values and SD of each variable were measured. There was a statistically significant difference between both groups. however according to ROC curves results, only the cut off value in patient group for CSA 0.13cm and AP diameter 2.82mm in distal forearm region yielded an optimum sensitivity 86.4% and 100% and specificity 100%. The limitation of this study is the relatively small number of patients and controls. Only patients diagnosed and confirmed by EDS were enrolled. It is known that having a typical clinical picture of CTS with negative EDS does not preclude the diagnosis of CTS. In addition no US parameters other than MN area and AP diameter were evaluated. Other potentially useful parameters are flattening ratio of MN and its echogenicity and mobility. US measuring AP diameter and CSA of MN before entering the carpal tunnel proved to be reliable and effective diagnostic tool in cases with clinically suspected CTS with high sensitivity and specificity in comparison to EDS. We advise the routine use of 2DUS in all clinically suspected cases of CTS especially with negative EDS finding.
