Abstract

Penile erection is a neurovascular process controled by psychological and hormonal factors in which nitric oxide (NO) is the principal neurotransmitter involved in erection at the molecular level,
Serum endocan is a novel proteoglycan secreted by endothelium of vascularized tissues which regulates the cell differentiation, migration, and adhesion. Current research suggests that endocan might have the key role in inflammation, healing, and tumorigenesis. Neovascularization associated with an inflammatory or a malignant process orderly showed elevated serum level of endocan and higher expression on affected tissue.
Endocan is considerd as a potential biomarker for both endothelial activation and dysfunction where it is believed to have specific mechanism where it is particularly related to inflamed endothelia.
Erectile dysfunction could to be a potential predictor of CV diseases in which endothelium dependent dysfunction may have a role. While endocan can be considered as an important endothelial marker and hence indicator of cardiovascular event.
The aim of this study was to study the correlation between serum endocan level and erectile dysfunction.
The present study was conducted on 45 male patients divided into two groups:
group A included thirty males patients complaining of erectile dysfunction which further subdivided into two subgroups:
group AI including fifteen diabetic patients with erectile dysfunction.
group AII Including fifteen non diabetic patients with erectile dysfunction.
group B included fifteen control subjects.
At the end of the experimentation the following results were reported:
The serum endocan level was statisticaly significant higher in patient group (A) than control group (B) with markedly elevated level in diabetic patients than non diabetics.
Also, there was asignificant negative correlation between serum endocan level and IIEF, where serum endocan level is higher in sever ED.
There was lack of correlation between serum endocan level and serum lipids. To evaluate the vascular status of the studied groups, CIMT was assessed. The CIMT was significantly higher
