Abstract

The study was done in EL MINIA university hospital internal medicine and diabetic out-clinic. In current study we included 120 subjects divided into three main groups 40 prediabetic patients, 40with type 2 diabetes mellitus diagnosed according to ADA 2015 and 40 control subjects. We classified them according FBG and glucose tolerance test.
Patients were subjected to clinical, abdominal ultrasonography, and laboratory evaluation which included: gylcemic parameters (FBG, 2hppBG, F.insulin, HbA1c, HOMA IR, HOMA B), Lipid profile(Cholesterol, triglycerides, HDL, LDL)renal function , Liver function and renal function.
The results of the study showed that, there was higher level of serum FGF21 in with hyperglycemic patients (prediabetic and diabetic) than control subjects (P <0.001).
Also FGF21 affected with treatments of the patients (antihypertensive in prediabetic patients),( antihyperglycemics in diabetic patients). This drugs lead to decrease level of FGF21.
Serum FGF21 correlated positively with age , systolic blood pressure , diastolic blood pressure , waist circumference , waist hip ratio ,BMI, FBG, 2hppBG ,F. insulin, HbA1C,HOMA IR ,T.Cholesterol, triglycerides, LDL,ALT,AST with P value (<0.001) for all of them except ALT P value (0.003).There was also negative correlation between FGF21 and HDL with p value (<0.001).
By multiple logistic regression analysis.We found that 2hppBG and triglycerides has a risk factor for incidence of hyperglycemia with p value (0.001,0.014 respectively).
When the following risk factors (age , systolic blood pressure , diastolic blood pressure , waist circumference , waist hip ratio ,BMI, FBG, 2hpp ,F. insulin, HA1C,HOMA IR, T. Cholesterol, Triglycerides, LDL,ALT,AST) were subjected to multiple linear regression analysis as independent factors for prediction of FGF21 in (pre-diabetic)group. We found that SBP, DBP, BMI, Triglycerides, HDL were significant independent predictors of FGF21 (p value=0.002, <0.001, 0.024, 0.002,<0.001, 0.002,<0.001 respectively).
