Abstract

In Egypt, there is no doubt that chronic liver diseases are a major health concern. Hepatitis C virus (HCV) prevalence among the 15-59 years age group is estimated to be 14.7%. Accordingly, Egypt has the highest HCV prevalence in the world. This unparalleled level of exposure to this infection appears to reflect a national level epidemic. It has been postulated that the epidemic has been caused by extensive iatrogenic transmission during the era of parenteral antischistosomal therapy mas treatment campaigns. Today, HCV infection and its complications are among the leading public health challenges in Egypt. Most of the cirrhotic patients display signs of cardiovascular disturbances that become more manifest following exposure to stress such as transplantation. Cardiac failure is an important cause of death following liver transplantation. It has been revealed that there is an overall association between the severity of the cirrhosis and the degree of cardiac dysfunction.This study was carried out to evaluate the right and left ventricular functions in patients with end stage liver disease candidate for liver transplant using pulsed tissue Doppler imaging.The study was conducted on 150 individuals and was classified into 2 groups:Case group: this group included100 patients with hepatitis C virus infection and presented with ESLD and candidate for liver transplantion. Control group: this group included 50 healthy subjects ; age and sex matched with the case group.After obtaining a written informed consents were obtained from the patients participating in this study or there relative. All patients were subjected to history including; personal history, disaese histodry and history of drug intake eg:antiarrythmic drugs, ribavirin, liver support,ect. Comparison of demographic data between group of patients and control group revealed no significant difference between the two studied groups as regard age and sex.In the current study there was no significant difference between the two groups regarding; LVEDD and LVESD. In the present study there is significant increased values of ejection fraction (EF), left atrial diameter (LAD) and AOD in the case group when compared with the control group. Left atrial diameter (LAD) and aortic root diameter (ARD) showed a highly significant increase in patients with cirrhosis.We found significant decreased values of MV E,MV A and MV E/A in the case group when compared with the control group (P-value <0.001).Our study found that E-wave DT is also one of the indicators of left ventricular diastolic function.We can conclude that systolic and diastolic functions of patients with ESLD candidate for liver transplant can be predicted with good sensitivity and specificity from Tei index estimated by TDI of lateral mitral and tricuspid annulus.
