Abstract

pipolar scissors is one of the new surgicalinstruments which has been recently usedin tonsillectomy and has received a considerable research interest.
Review of literature revealed different techniques for tonsillectomy which have been studied thoroughly, but convincing evidence in literature is still lacking as far as the optimal method of performing this surgical procedure.
In this work, fifty patients with recurrent attacks of tonsillitis, and were indicated for tonsillectomy, were divided into two equal groups. The first group underwent bipolar scissors tonsillectomy, and the second one underwent the cold dissection technique.
According to our statistical analysis and data, bipolar scissors technique proved to be much better than the cold dissection tonsillectomy regarding the easiness of use, reduced intraoperative blood loss, operative time and postoperative hemorrhage which indirectly impacts the cost-effectiveness of this procedure. The median intraoperative blood loss was 30.0 (20 - 50) ml with the bipolar scissors while with the cold dissection technique it was 75.0 (60 - 100) ml. it has been found that the mean operative time was 22.20 ± 9.37min with the bipolar scissors and it was 28.36 ± 11.19min with the cold dissection group.Also, the incidence of postoperative hemorrhage was higher with the cold dissection (8% - 2 cases) than the bipolar scissors technique (4% - 1 case).
The disadvantages of the bipolar scissors include delayed postoperative pain, and thermal trauma to the tongue and oral mucosa. It showed a higher mean overall pain score of 13.84 ± 2.64, if compared with 8.08 ± 3.29of the cold dissection technique.
In our study, we concluded that bipolar scissors is proved to be safe and effective measure for tonsillectomy.
