Abstract Subtrochanteric fractures account for a proximately 10-34% of all hip fractures.(155) The treatment of these fractures continues to present a challenge to orthopedic surgeon due to anatomical and biomechanical features including associated comminution concentration of high stresses and strong deforming forces due to muscular attachment.(159) In adults, non-surgical treatment has little or no place due to high rate non-union, delayed and malunion and complications of prolonged recumbency. Extramedullary devices showed limited success with frequent implant failure, excessive blood loss non-, and malunion. On the other hand, intramedullary devices are reliable implants for subtrochanteric fractures. They have high rates of union with minimal soft-tissue damage and less complications. Intramedullary fixation has biological and biomechanical advantages, but surgery is technically demanding. Gradual learning and patience is needed to make this method truely rewarding.(173) The aim of this work was to evaluate the results of treatment of subtrochanteric femoral fractures in adults by using Trigen Tan intramedullary nail. This study included 21 patients with traumatic subtrochanteric fracture of the femur. The age of the patients ranged from 19 to 50 years with a mean value of 41.76 ± 9.69 years. Twelve patients were males (57.1%) and nine patients were females (42.9%). The mechanism of injury varied, seven fractures were caused by road traffic accident (33.3%), 11 fractures (52.4%) caused by simple fall at home and three fractures (14.3%) were caused by falling from a height.
