Abstract

The appearance of enamel demineralization areas or white spot lesions is a significant risk associated with fixed orthodontic treatment. The aim of the present study was to evaluate and compare effect of different mineralizing agents in prevention and treatment of white spot lesions around orthodontic brackets. The study was carried on one hundred-twenty extracted human maxillary premolars. The teeth were divided randomly into two main equal groups. group I (n=60) considered as the treatment group while group II (n=60) considered as the prevention group. Then, the teeth in either group were subdivided into four equal subgroups according to the mineralized agent utilized. These materials were Profluorid varnish, Clinpro white varnish, Apa care HA, Ghimas n-HA and they were applied according to manufacturer’s instructions. In the treatment group, square window (4x4mm) in the middle of the buccal surface of the teeth was determined and the remaining buccal surface was painted by nail polish. Then the teeth were immersed in solutions cycle for production of enamel decalcification (WSLs) in the preformed window.Meanwhile, in the prevention group, metal orthodontic brackets were bonded to the buccal surface of all teeth. Enamel minerals were assessed utilizing laserfluorescence. Also enamel Ca and P levels were assessed using environmental scanning electron microscope. In the treatment group measurements were done at five sessions; before and after WSLs production, and after 1st, 2nd, and 3rd month of mineralizing agents’ application. Meanwhile, in the prevention group measurements were done at four sections; after bracket bonding, and after 1st, 2nd, and 3rd month of mineralizing agents’ application. The results revealed that; in the prevention group, all the studied mineralizing agents were effective in prevention of enamel decalcification as there were no significant differences between DIAGNOdent measurements and Ca and P levels after bracket bonding and after 1,2 and 3 months of mineralizing agents application (P>0.05). While in the treatment group, all studied materials were effective in enamel remineralization as there were significant differences between DIAGNOdent measurements and Ca and P levels before enamel decalcification and after 1, 2 and 3 months of application of the studied agents (P<0.05). Ghimas n-HA revealed the highest effect in remineralization while Profluorid varnish showed the least remineralizing effect.However there were no significant differences between the effects of n-HA, Clinpro white varnish and Apa care HA. For each material there were no significant difference between 1, 2 and 3 months of mineralizing agent application(P>0.05).
