Abstract

Background: Antiepileptic drugs are one of the most important factors that may affect bone health. They are associated with significant side effects including, but not limited to, radiological evidence of rickets, decreased bone mineral density (BMD), altered bone turnover, and increased risk of fracture.
Aim of the Work: The aim of the present study is to investigate the effects of antiepileptic drugs on bone mineral density and bone turnover markers.
Patients and Methods: This is a case control study which was conducted on eighty children aged from 6-12 years recruited from Children’s Hospital, Ain Shams University. Controls: Forty apparently healthy children with age and sex matching to those of group 1 were enrolled as controls; they were recruited from the relatives and siblings of the children attending the Outpatients’ clinic.
Results: Our study included 40 children with idiopathic epilepsy, their ages ranged from 6-12 years, they included 22 males and 18 females, and the mean duration of treatment with AEDs was 4.23 ± 1.15 years ranged from 3-8 years. Our studied patients were classified regarding antiepileptic drug therapy into 17 patients on Polytherapy (42.5%) and 23 patients on monotherapy (57.5%) as follow; 8 patients on levetiracetam (20.0%), 8 patients on valproate (20.0%), 7 patients on carbamazepine (17.5%). The results of our study showed high significant lower BMD of lumbar spine in patients compared to the controls (p<0.001) and higher levels of C-terminal telopeptide of type 1 collagen (CTX) and alkaline phosphatase in patients compared to the controls (p<0.001). Also there is high significant difference between patients and controls groups regarding Ca level being lower in patients compared to the controls (p<0.001) but no significant difference between patients and controls groups regarding phosphorus level.
Conclusion: In our study we found that antiepileptic drugs (AEDs) have an effects on bone health and metabolism through change in both of bone mineral density (BMD) and bone turnover markers.
