Abstract

The aim of this study was to assess systolic and diastolic right
ventricular function of patients with β-TM using Echo-Doppler imaging, including tissue doppler imaging modality (TDI) and to determine whether this modality is an adequate diagnostic tool for the screening and detection of subclinical cardiac dysfunction in β-TM.
This is a case-control study that included 40 patients with β-TM who were following up regularly in the outpatient clinic of hematology in Beni Suef University Hospital , and 40 ages and sex matched subjects taken as control. All patients were subjected to full history and through clinical examination with stress on: age at diagnosis, frequency of blood transfusion, chelation therapy type, dose and compliance to therapy. Serum ferritin as well as hemoglobin level of the patients at the time of study enrollment were also recorded.
Transthorathic resting echocardiography was recorded in patients with
β-TM and in normal controls, where M-mode and two conventional echocardiography were performed and measurements were obtained. PWD echocardiography and TDI, to determine RV function and to calculate the myocardial performance index, were performed as well. Cardiac dimensions, systolic and diastolic functions of the RV were obtained and values in β-TM patients and in normal controls were compared.
Evaluation of RV diastolic function was done by measurement of E and A wave velocities across the tricuspid valve, E/A ratio and DT. MPI of RV was calculated using PWD and TDI to evaluate the RV systolic function.
The evaluation of thalassemic patients by different echocardiographic modalities revealed the following:
- Conventional echocardiography revealed statistically significant increase in RV and RA dimensions in patients in comparison with the control group.
-There was no significant difference in LV EF denoting delayed affection in LV systolic function among cases.
- Regarding RV systolic function, TDI derived MPI was significantly higher in patients group in comparison with control group with P value (0.000) however PWD showed no significant difference in RV MPI between both groups with P value (0.487).
- Regarding RV diastolic function, there was no significant difference in the E/A ratio between the two study groups. PWD also showed that deceleration time (DT) was significantly lower in the patients in comparison with the controls with P value (0.000), however the mean value of DT was within the normal range.
- There was a strong significant correlation between the serum ferritin level and TDI derived RV MPI.
