Abstract This study was conducted in order to eva lu ate serum concentra ti on of pro cal citonin in the newborn infants as a predictor of neon atal sepsi s, and its value as a pararr.eter to follow up septic inflamm ati on and to estimate progr1osis and success of th erapy . For this purpose, 40 newborn infants presenting with symptomps and signs suggesting neonatal sepsis admi tted to Neonatal Intensive Care Unit of Mansoura Gen eral H0spital (20 of them ha ve positive blood cu lture resu lts, wh:;e the rem ainin g had negative cu lture resu l ts) were en rollee! in the stud y . In add ition, 20 health y newborn infants of comparable age and sex were included as controls. A ll the study population were subjected to history tDk ing, clinica l examination , compl ete blood coun t (tota: and differenti al l eukocytic count, liT & JIM rati os of granulocytes, and platelet count), erythrocyte sedim entation rate (ESR), qualitative m easurement of CRP, X-ray chest for cas(:.S w ith respiratory di stress, blood cu ltu re, CSF examination and cultu re for cases w ith su spected m eningitis. Quantitation of plasma procalcitonin (Pro-CT) level by Radioimmunoassay was done. Two consecutive samples were obtained for both patient groups; the first sample was obtained at the time of diagnosis of sepsis, and the second follow up sample was obtained Iweek later. The majority of cases of neonatal seps1s, were without localization, some had pneumonia, meningitis, and others with skin infection. Respiratory distress and .lethargy & poor suckling and mottling were the commonest presentation of sepsis in our study, followed by GIT manifestations and convulsions, while apnea was the l east presenting symptom of sepsis. Regarding laboratory diagnostic criteria of neonatal sepsis, normal total leukocytic count was a common finding, some had leukocytosis, others had leukopenia. Bandemia ( 400/mm 3 was found in 17 cases (42.5%), while 23 cases (57.5%) had normal band cell count. liT ratio 0.2 was found in 16 cases (40%), 1/M ratio 0.3 in 12 cases (30%).
