Abstract The DME is a complex disease of multifactorial origin, the first line treatment is the hyperglycemia control, accompanied by monitoring of blood pressure and lipid levels, which permits the disappearance of macular edema in about 33% to 35% of patients, in the other 70% of cases we must establish a treatment based on laser photocoagulation or intravitreal injections of anti-VEGF. The main purpose of this prospective, descriptive and interventional study was to investigate the effect of focal laser treatment on the vitreomacular interface relationship. In the current study, Focal laser photocoagulation was performed for all patients who suffered from clinical significant diabetic macular edema either non center involving or center involving who first received intravitreal anti VEGFs injections. These included 40 eyes of diabetic patients with clinical significant macular edema between 18 to 65 years of age; 20 eyes with center involving DME who had received intravitreal anti VEGFs injections before the laser therapy and the CST dropped to less than 320 microns in men and 305 microns in females and 20 eyes with non-center involving DME. BCVA, FA and OCT were obtained before intervention, Patients were followed up after 3 months and 6 months post laser treatment, visual acuity was noted and OCT was done. Results showed that 10% developed vitreomacular traction in the form of fine ERM and nearly 90% had no change in their vitreomacular interface relationship. 50% maintained their visual acuity level, 40 % showed deterioration in their visual acuity and 10% showed improvement in their visual acuity. There are few complications in laser treatment when it is carried out correctly, but it is important to take into consideration the increase in retinal scar size over time. |
