Abstract

Rheumatic valvular heart disease remains the most common acquired heart
disease worldwide. It’s the major cause of cardiovascular death in pediatric age
group, so an accurate diagnosis of RF is very important.
The present study was done to detect the prevalence of rheumatic heart
disease among the primary and preparatory school children 6-15 years old in Al-
Shohadaa district, Menoufia Governorate in Egypt.
Screening survey was done on 5000 children (2578 males and 2422 females)
in 51 governmental, 3 private primary schools and 32 governmental, 2 private
preparatory schools in Al- Shohadaa district.
Careful cardiovascular examination was done for all students and any child
with audible heart murmur was subjected to the following:
Full history taking, thorough clinical examination, laboratory investigations (CBC, ESR, CRP and ASOT) and Echo Doppler study.
It was found that:
135 students (2.7%) were found to have abnormal history or examination
indicating to perform Echocardiography, 76 of them (1.52%) receive long acting
penicillin, 15 cases (0.30%) with family history of rheumatic heart disease, 44
cases (0.88%) discovered murmur on auscultation.
The echocardiographic findings among 124 selected students which were
(1.6%) definite RHD, (0.8%) borderline RHD with the prevalence rate of
rheumatic valvular heart disease in primary and preparatory schools children in Al-
Shohadaa is found to be is 0.06% (3/5000).
3 children of them have rheumatic valvular heart disease. They were 2 males
(66.7%) and 1 female (33.3%), 2 cases (66.7%) were from preparatory rural
schools and 1 case (33.3%) were from primary urban schools. With the majority of
them (80%) have rheumatic heart disease were of low socioeconomic state, 2
students with low Socioeconomic level (66.7%) and 1 student with middle
(33.3%).
In this study we found the v alve lesions were as follows:
1 case (1.6%) definite RHD, 2 cases (0.8%) borderline RHD, 12 cases
(9.7%) mitral valve regurgitation not meeting criteria of RHD, 3 cases (2.4%)
aortic regurgitation not meeting criteria of RHD,4 cases(3.2%) congenital heart
disease.
