Abstract

Equinovalgus is the most common deformity of the foot seen in children with cerebral palsy. Spasticity and tightness of the calf and peroneal muscles are the main causes. A tight triceps surae causes a bowstring effect across the ankle and subtalar joints. This is the result of muscle imbalance which displaces the hind foot into valgus. Spasticity of the calf and peroneal The aim of this study is to: evaluate the results of extra-articular subtalar arthroereisis in the treatment of equinovalgus deformity in children with spastic cerebral palsy, as well as to detect the factors affecting the results and the complications. Patients & methods: In the period between November 2015 and October 2017 a prospective study was carried out on 16 spastic cerebral palsy children with equinovalgus deformity in 27 feet. All cases were selected from the pediatric orthopaedic outpatient clinic at Mansoura University. In our study, there were 8 boys and 8 girls. Eleven children were bilateral and five were unilateral. The average age at the time of surgery was 6.8 years. Recommendations: Extra-articular subtalar arthroereisis corrects moderate to severe hindfoot valgus in ambulatory children with spastic cerebral palsy (CP) and achieves foot alignment, improving pain and skin complications and avoids the arthrodesis problems. A long-term follow-up with a larger patient population is recommended to detect any recurrent or residual hindfoot deformity.
