Abstract

Skin and nervous system arise from a common embryological origin: the embryonic ectoderm. This may be the reason by which the skin gives an account of our mental and emotional state through infections and injuries. This is illustrated by the fact that up to 33% of dermatologic patients have concurrent psychiatric disorders or dysfunctional psychosocial aspects.
Psycho-cutaneous disease, defined as primary psychiatric disease with skin manifestations, is frequently encountered in dermatology, with 30% prevalence of psychiatric comorbidity in the outpatient of dermatology. Patients with psycho-cutaneous disease routinely refuse mental health resources, leaving the burden of care upon the dermatologist.
Obsessive compulsive disorder is one of the most common Psycho-cutaneous disease.The prevalence of OCD in a dermatological practice may be much higher than in the general population. A significant number of patients with undiagnosed OCD appear to present to dermatology clinics. There may be a common etiological factor underlying the relationship between OCD and some dermatological conditions such as; eczema and acne. Identification and referral for psychiatric consultation may aid in treatment of both OCD and concomitant dermatological conditions.
Obsessive compulsive disorder is a psychiatric condition characterized by persistent and unwanted intrusive thoughts, images and urges (obsessions) and repetitive behaviors or mental acts (compulsions). OCD present in dermatology in many forms as skin excoriation, frequent hand washing, trichotillomania, onychotillomania and body dysmorphic disorder. The aim of this study was to determine the prevalence of obsessive-compulsive disorder in dermatological patients attending Mansoura University Hospital, the different dermatological presentation of the OCD and the possible relationship between dermatological lesions and the severity of OCD. The suspected patients were assessed with DSM-V criteria and The Y-BOCS questionnaire for assessment the severity.
The current study was conducted on 500 dermatological patients. It was found that the prevalence of OCD in this study was (10.6%) according to the DSM-V criteria for OCD. The OCD spectrum disorders distribution in OCD group are, 2(3.8%) with skin excoriations, 14(26.4%) with trichotillomania, 2(3.8%) with body dysmorphic disorder, 0(0.0) with hoarding disorder and 37(69.9%) with OCD. The distribution of OCD symptoms are 22(59.5%) with fear of contamination, 10(27.03%) with onychotillomania, 3(8.1%) with checking, 1(2.7%) with counting and 1(2.7%) with religious
The result of Y-BOCS questionnaire score in the OCD group the median was 13 with minimum 5 and maximum 32.which meet the criteria of mild OCD
