Abstract

The current study is case-control, prospective, observational, comparative and one center study. It is conducted at cardiology department, Mansoura specialized hospital, on patients patient referred for diagnostic coronary angiography in the period from January, 2014 to November, 2016 and aimed to evaluate the prevalence of soft tissue rheumatism in relation to severity of coronary artery disease. The study included 420 patients (100 with normal coronary angiogram, 100 with non-obstructive coronary CAD and 210 with obstructed CAD). They were 269 males and 151 females, with mean ages 55.74 + 10.09 (25- 80 years). Results: Significant increase in mean age and positive age risk (57.5±10.0), (71.7%) among obstructive CA, than both normal CA (52.3±10.1), (39.0%) and non-obstructive CA (55.3±9.13), (58.3%) groups. Significant increase in male gender (68.6%) among obstructive CA than both normal CA (60.0%) and non-obstructive CA (58.0%). Non significance difference in inhabitance between the obstructive CA, normal CA and non-obstructive CA group. Significant increased prevalance of previous MI (16%) among obstructive CA. than normal CA (7%) and non-obstructive CA (4%). significant increase numbers of risk factors among obstructive CA, than normal and non-obstructive groups. Non significance difference between the obstructive CA, normal CA and non-obstructive CA groups as regard other individual risk factors (HTN, DM, smoking, dyslipidemia and family history). Significant increase in prevalence of typical chest pain, mild dyspnea, fatigue, effort and sweating among obstructive CA, than normal CA and non-obstructive CA group. Significant increase in prevalence of anti-ischemic drugs usage among obstructive CA (100%), than normal CA (80.0%) and non-obstructive CA (98%). Non significance difference between obstructive CA, normal CA and non-obstructive CA groups as regard usage of (anti-ulcer, anti HTN, anti DM, lipid lowering agents and anti-rheumatics) medications. Non significance difference between obstructive CA, normal CA and non-obstructive CA groups as regard general(pulse, systolic BP and diastolic BP), chest wheez and cardiac examination (LL edema, pallor). Conclusion: Soft tissue rheumatism is more prevalent in normal CA more than obstructive CAD. The most prevalent soft tissue rheumatism pattern was front chest affection (titez syndrome and costochondritis. Soft tissue rheumatism is more prevelant in upper body more than lower body. Soft tissue rheumatism is more prevalent in rural areas more than urban. Soft tissue rheumatism is more prevalent in young population.
