Abstract

Insulin-like growth factor (IGF) system plays critical roles in somatic growth in an endocrine fashion as well as proliferation and differentiation of normal and malignant cells in aparacrine \ autocrinefashion it enhancing the cell proliferation,differentiation and it is a potent inhibitor of apoptosis . The liver is a major site of IGF-1 production under control of growth factor and.the bioavailability of IGFs is modulated by high affinity binding proteins known as insulin-like growth factor binding proteins (IGFBP3)(from 1-7) .Most of the IGF-1 in circulation bound to IGFBP3 of which the liver is a significant source).
Reduction of serumIGF1 also plays a role in development of insulin resistance in HCV-patients and is associated with development of HCC from cirrhosis .The aim of this study is to investigate the status of serum levels of IFG-1, IGFBP-3 and IFG-1/ IGFBP-3 molar ratio in patients with HCV-induced cirrhosis with and without HCC compared to healthy subjects as controls. Also aimed to find out a possible diagnostic ability of these three variables in the prediction of HCV-induced HCC.
This case control study included 50 patients with HCV related liver cirrhosis complicated with HCC, 30 patients with liver cirrhosis without HCC and 30 healthy peoples who were age and sex matched as a control group. Serum IGF-1, serum IGFBP-3, plasma insulin were quantitatively measured by ELISA methods. Homeostatic model for assessment of insulin resistance (HOMA-IR) and IGF1/IGFBP-3 molar ratio were calculated.
