Abstract

This work aims to study the role of DKK-1 in the pathogenesis of chronic lymphocytic leukemia and its prognostic value in the disease.The study was carried out on seventy-five subjects; fifty CLL patients who were selected from Minia Oncology Center and Minia University Hospital, from August 2016 to February 2017 and 25 apparently healthy subjects as controls.
group (І) was twenty-five subjects who were newly diagnosed CLL patients, group (IІ) was twenty-five subjects who were on treatment for more than one year and group (ІII) were twenty-five healthy subjects as a control.
All patients and controls were subjected to:
1. Complete blood count (CBC), ESR, renal and liver function tests, serum LDH and uric acid.
2. Special investigations: Serum DKK-1 by ELISA method.
The results of this study were summarized as follows:
Hb was significantly lowered in group І and group II when compared with that of control group (p-value <0.001 in both)
HCT was significantly lowered in group І and group II when compared with that of control group (p=0.009, 0.024 respectively).
MCV,MCH and platelet were significantly lowered in group II when compared with that of control group (p=0.006, 0.002 and 0.001 respectively).
There was significant decrease in platelet count of group II when compared with control group. Also there was significant decrease found on comparing group II to group I (p-value < 0.001 in both).
WBCs (total count) was significantly elevated in group І, II when compared with control group (p=<0.001, <0.002 respectively) and there was a significant increase found on comparing group I to group II (p-value< 0.001).
ESR was significantly elevatedin group I, II when compared with control group (p–value <0.001 in both) and significant increase found on comparing group I to group II (p-value< 0.001).
LDH was significantly elevated in group I compared to group II and control group (p-value <0.001 in both) and there was statistically significant increase was found on comparing group I and group II with each other (p-value <0.001).
Uric acid was significantly elevated in group I compared to group II and control group (p-value <0.001 in both).
In the present study, DKK-1 was significantly decreased in group I compared to group II and control group (p-value 0.003, <0.001 respectively) and there was no statistically significant difference on comparing group II and control group with each other (p-value 0.114).
There was significant positive correlations between TLC in group II and DKK-1 (r=-0.460, p=0.021).
There were significant positive correlations between 1st hour ESR in group I ,II and DKK-1 (r= - 0.538, p=0.006, r= - 0.415, p=0.039 respectively).
