Abstract

The present study was done to induce AP by l- arginine in adult male albino rats, evaluate the effect of H2S, leptin and curcumin against acute pancreatitis induced by l-arginine and to show the most effective drug in AP.
The study was conducted on 80 adult male rats, weighing (250 –300 gm.). They were housed in wire mesh cages at room temperature with natural light/dark cycle. They were left for two weeks after arrival from the supplier for acclimatization. Rats were fed a standard diet of commercial rat chow and tap water adlibitum through the time of the study.
Conclusion:
- L- arginine in a dose 2.5 gm./Kg resulted in AP in a model of rat which is characterized by increase in the serum levels of pancreatic enzymes (amylase and lipase), NO and TNFα and pancreatic MDA. In addition, disruption of the pancreatic architecture, significant interstitial edema, acinar cell necrosis, extensive inflammatory cell infiltration mainly neutrophils and few areas of hemorrhage which were revealed by pancreatic histopathology. It induced AP due to the excess generation of ROS, decrease in lysosomal degradation process, lysosome-associated membrane protein-2 (LAMP-2), mitochondria damage, elevation of pancreatic IL-1β level and the excess NO production.
- H2S was efficient to counter act the inflammatory changes produced by l-arginine and bring them back to insignificant level from control group. This effect may be due to inhibiting PI3K, neutrophil activation, reduction of the leucocyte infiltration, the NO production and free radicals.
- Administration of leptin or curcumin partially corrected the inflammatory changes but cannot reach to control group.
- H2S was better than leptin or curcumin in controlling the inflammatory changes due to prevention of leukocytes adhesin to the endothelium and their migration into the interstitial space as a result of activation of ATP sensitive K+ channels on leukocytes and endothelial cells. Thus, H2S causes more limitation of inflammation than leptin and curcumin.
- Leptin was more potent in controlling the inflammatory changes than curcumin due to the stimulatory effect of leptin on α MSH and glucocorticoid release and the increase in the gene expression of HSP60.
Limitation:
- Serum levels of pancreatic trypsin, total antioxidants and corticosteroids were not measured due to high cost.
Recommendation:
- Future researches should be directed to study the potential benefit of combining both leptin and curcumin or to use other regimen natural.
- Future researches should be also directed to know the underlying mechanism through which the H2S acts whether through CO or prostaglandin.
