Abstract

Twelve articles out of twenty were chosen based on our inclusion and exclusion criteria providing the needed tools for accurate comparison between different tools regarding mean age of patients in each study,follow up period, rate of union ,rate of nonunion ,rate of AVN and reoperation rate.The highest union rate was achieved by Targon femoral neck device.followed by primary valgus osteotomy followed by medial buttress plate augmentation of cannulated screws followed by other methods and the lowest rate of union was achieved by cancellous screws fixation.thereʾs high statistically significant association between union rate and method of fixation The reoperation rate was highest with fixed angle device application followed by both cannulated and cancellous screws fixation by the same percentage followed by Targon femoral neck device and lowest with medial buttress plate augmentation. thereʾs high statistically significant association between reoperation rate and method of fixation The AVN rate was highest with fixed angle device followed by Targon femoral neck device and lowest with medial buttress plate augmentation of cannulated screws but thereʾs no static significance had been found between AVN and method of fixation
Regarding fixed angle devices , 130ºdouble angled blade plate achieved highest union rate than other methods while Dynamic hip screw achieved highest rate of AVN but thereʾs no statistically significant association had been found between union rate, AVN rate and the type of fixed angle device used.
Open reduction and internal fixation with cancellous screws achieved higher union rate ,lower reoperation rate and slightly lower AVN rate than the closed method with cancellous screws.thereʾs high statistically significant association between union rate and method of reduction , statistically significant association between reoperation rate and method of reduction and no statistically significant association between AVN rate and method of reduction.
In the future ,comparative studies will be done on different methods of femoral neck fixation including biomechanical studies on application of medial buttress plate augmentation and Targon femoral neck device to compare between them and tradirional methods of fixation.
